Painless accumulation of fluid inside the sac around the testis.

Etiology

  1. Congenital Hydrocele
    • Due to patent processus vaginalis.
      • Peritoneal fluid flows into the scrotum ( Communicating Hydrocele )
      • Peritoneal cavity may not be in direct communication with the scrotum ( Non – Communicating Hydrocele
  2. Acquired Hydrocele
    • Due to Trauma, Tumour, Torsion, infection ( Wuchereria bancrofti )
  3. Idiopathic causes

Clinical Features

  • Fluctuant painless swelling
  • Positive transullimnation

Diagnosis

  • Clinical Diagnosis
  • Ultrasound – Hypoechoic Fluid

Differential Diagnosis

  • Testicular Tumour
  • Inguinal Hernia

Treatment

Congenital Hydrocele – Resolves within 6 months

Surgery

  1. After six months without resolution
  2. Discomfort
  3. Infertility

Procedures

  • Hydrocelectomy
    • Percutaneous aspiration of the fluid
